Medical Records
To receive a copy of your medical records the following is required: - Sign a release of information. (form is available below) - Provide photo I.D. at the time you pick up your records. - Must be at least 18 to acquire personal records, otherwise a legal guardian signature is required. - Once a minor turns 18 a parent may not pick up the patient's records. - Please give 24 hours notice that you would like a copy of your records by contacting the Medical Records Depratment at 784-1221 .
