What is Post-Acute Rehabilitation?

This program is designed for patients who have had a 3-overnight hospital stay and need to increase strength and independence before returning home. This short-term program provides skilled treatment to help you reach specific goals for self-care and mobility. You are the center of a multi-disciplinary team that can include:

  • Physicians
  • Nurses
  • Case Manager / Discharge Planner
  • Pharmacist
  • Dietician
  • Physical, Occupational, and Speech Therapists
  • Activities Coordinator

Even if your surgery or hospital stay was not at SMC, you can transfer here to receive rehabilitation and be close to your home, family and friends.

  • Decreases burden on family associated with transportation, food and lodging costs for travel.
  • Links to community resources in patient’s own community.

How do I get admitted to this program?

  • Call and speak with one of our Case Managers at (208) 784-1221, Ext. 339.
  • If you are planning a surgery such as hip or knee replacement, heart or lung surgery and know you will be in admitted hospital status for at least 3-nights you may qualify to rehabilitate closer to home. Call and we can help to coordinate.
