How do I get a copy of my Medical Record?

1. Sign a release of information (form is available below or by calling (208) 784-7017, Ext. 314). Please make your request 24 hours in advance of picking up your records.
2. Must be 18 years of age to acquire personal records, otherwise a legal guardian’s signature is required.
3. Once a minor reaches the age of 18, the parent may not pick up the patient’s records. Provide a photo ID at the time you pick up the record.
